Pelvic health isn’t just a women’s issue.

Men experience pelvic floor dysfunction too, and it’s more common than you might think. As a physiotherapy clinic with specialized training in male pelvic health, we provide a comfortable, respectful, and confidential environment to address concerns that can often feel difficult to talk about.

The pelvic floor muscles play a key role in bladder and bowel control, sexual function, and core stability. When these muscles aren’t working optimally, it can lead to issues such as urinary leakage, urgency, pelvic pain, erectile dysfunction, or post-prostate surgery challenges.

Treatment is always tailored to you and may include education, targeted exercises, manual therapy, breathing and coordination strategies, and practical lifestyle guidance. Our goal is to help you regain control, reduce symptoms, and get back to living confidently and comfortably.

  • Incontinence
  • Constipation
  • Pelvic Floor Muscle Tightness – Overactive or tense muscles that make passing urine painful or difficult.
  • Post-Urination Discomfort – A burning, aching, or throbbing sensation in the penis, perineum, or pelvis after emptying the bladder.
  • Non-Bacterial Prostatitis / CPPS – Chronic pelvic pain that feels like a prostate infection, but is actually related to muscle tension and nerve irritation.
  • Hesitancy or Straining – Feeling the need to push or strain to start or finish urinating.
  • Pain with Ejaculation – A burning, sharp, or aching pain in the penis, perineum, or pelvic floor during or immediately after climax.
  • Deep Pelvic Aching – A dull, persistent ache in the pelvis, rectum, or genitals that can last for hours or even days after sexual activity.
  • Discomfort with Erections – Pain or a feeling of tight restriction as the penis becomes erect, often linked to pelvic floor tension.
  • Prostate or Testicular Pain – Discomfort in the groin or testicles during arousal or intercourse that isn’t caused by an active infection.
  • Difficulty Maintaining Erections – Weakness in the pelvic floor muscles can make it hard to sustain an erection, as these muscles are responsible for restricting blood outflow.
  • Changes in Erection Quality – Erections that feel less firm or rigid than usual due to poor muscle tone or blood flow.
  • Post-Prostatectomy Recovery – Erectile issues following prostate surgery, where specialized physiotherapy helps retrain the nerves and muscles to recover function.
  • Hard-Flaccid Syndrome – A condition where the penis feels semi-rigid or firm even when completely flaccid, often caused by chronic pelvic floor tension and anxiety.

F.A.Q.s about Pelvic Health Physiotherapy

Pelvic Health physiotherapy is a unique service meant to address issues involving the inside of the pelvis. This is a regulated service where special training has been provided to assess the internal muscles, organs, nerves and tissues. The Gold Standard for pelvic health assessment and treatment includes internal rectal examination, but also, only performing whatever is comfortable for the patient to address their concerns and meet their goals. At Calgary Core Physio, we are proud to be able to offer Men’s Pelvic Health Physiotherapy to address common, but often neglected, pelvic health conditions.

A thorough medical history is gathered during assessment. Afterwards the outside of the body is assessed. This includes functional and biomechanical assessment of the back, exterior pelvis, hips, abdominal muscles and recruitment patterns, and breathing patterns. Next, the exterior and interior of the pelvis and its structures are assessed. Your therapist will evaluate muscle tone, muscle length, pain response, ability of these muscles to contract and relax, and motor strategies used.

Typically, pelvic health assessments are not painful. Sometimes, however, certain structures or muscles can be painful either due to muscle tightness (think pushing on a muscle knot) or feedback from the brain. Communication with your therapist is ongoing to ensure comfort throughout assessment and treatment.  All pelvic health assessments and treatments are conducted in private rooms in order to ensure privacy.

  • Hands on treatment both inside and outside of the pelvis
  • Intramuscular stimulation and/or acupuncture
  • Bladder retraining
  • Education regarding management of bowel and bladder habits
  • Core retraining involving breath work and abdominal muscles
  • Progressive exercises to achieve movement and fitness goals
